We are open Mondays through Fridays from 8-5.
Our center welcomes patients who are self-referred as well as patients who are referred by their physicians.
Insurance
If you would like to find out if your insurance covers the heart scan, you need to send us an order from your doctor along with prior authorization from your insurance provider. Your provider must obtain the prior authorization from your insurance provider and forward it to us. Front Range Preventive Imaging cannot do this for you.
Paperwork
Because of the nature of screening procedures, certain paperwork is necessary. We are pleased to offer you the opportunity to print out the appropriate paperwork for your scan here, fill it out at home, and bring it with you at the time of your appointment. Due to patient confidentiality, we cannot have paperwork filled out on line.
